Syllabus Statement

The University of Utah and the Utah System of Higher Education (USHE) require instructors to include specific information about the institution and their courses in all published syllabi (Syllabus Design).  To comply with policy and inform students of the University's accommodation policies, the following statement should be included in all course syllabi (ADA Statement):

Americans With Disabilities Act (ADA)

The University of Utah seeks to provide equal access to its programs, services, and activities for people with disabilities.
All written information in this course can be made available in an alternative format with prior notification to the Center for Disability & Access (CDA). CDA will work with you and the instructor to make arrangements for accommodations. Prior notice is appreciated. In compliance with ADA requirements, some students may need to record course content. Any recordings of course content are for personal use only, should not be shared, and should never be made publicly available. In addition, recordings must be destroyed at the conclusion of the course.

Additionally, the following statement regarding field experience, clinical placements, practicum, internships and extenships, should be part of an expanded syllabus, when applicable:
If a student needs to request disability-related accommodations for a field experience, clinical placement, practicum, internship, externship, or other comparable work activity for academic credit, that student must make their request through the Center for Disability and Access (CDA).
As with academic accommodations, the CDA engages in an interactive process with the student and all stakeholders to determine reasonable accommodations based on the unique requirements of the experience. As part of this process, CDA may disclose an accommodation and parameters of that requested accommodation but will not disclose a student's diagnosis or health condition related to those accommodations.
