My student is not registered with the CDA, but should be.

  

The student needs to complete an intake form and schedule an appointment with a consultant.

My student is not a CDA student and is not showing up to class nor responding to my emails.

Fill out a student referral report with ODOS.

Health, Injury, or Surgery

Refer to the CDA

For students with chronic health conditions, who are unable to finish the semester, experienced an injury, or have any other medical reason to require accommodations, please refer the student to the CDA.

 

 

Disability

Refer any student with a temporary or permanent disability to the CDA.

 

Safety Concerns (including food insecurity)

Access to food, clothes, or other resources

Basic Needs Collective 

Fill out a report for a student of concern or a student behavior report
