Health & Wellness Resources
My student is registered with the CDA.
Contact the Accessibility Consultant.
If you do not know who the student's Accessibility Consultant is, you can find it on the accommodation letter/email.
My student is not registered with the CDA, but should be.
The student needs to complete an intake form and schedule an appointment with a consultant.
Please do not authorize accommodations in your course for students who are not registered with the CDA.
My student is not a CDA student and is not showing up to class nor responding to my emails.
Fill out a student referral report with ODOS.

Refer to the CDA
For students with chronic health conditions, who are unable to finish the semester, experienced an injury, or have any other medical reason to require accommodations, please refer the student to the CDA.
Disability
Refer any student with a temporary or permanent disability to the CDA.

If you have a student who comes to you needing support, the Center for Campus Wellness is a great place to start. They share the suite with the CDA! Come visit all of us at the Student Services Building room 350.

University Counseling Center
