Eligibility and How to Qualify for Accommodations

The Center for Disability & Access is the designated office on campus that determines student eligibility for accommodations at the University of Utah.

The following are the steps student should take to become eligible:

Step One: Intake Form

Complete a New Intake Form.

(You will receive an invitation to schedule an intake appointment within 2 business days of completing the form).

Step Two: Schedule an Intake Appointment

Schedule an in-person or virtual intake appointment through the Student Online Portal to meet with an Accessibility Consultant.

Step Three: Submit Documentation

Submit documentation as part or the new intake form or at a later point through the Student Online Portal.  (Documentation Guidelines)

Documentation may include a completed Disability Verification Form, letter from a medical or psychological provider about the condition, psychoeducational or neuropsychological evaluation, a 504 plan, Individualized Education Plan (IEP), or other history of accommodations. 

Students should not let the lack of documentation prevent them from visiting with an Accessibility Consultant. The Accessibility Consultant can help find resources on campus and in the community to obtain documentation.

Step Four: Meet with an Accessibility Consultant

Meet with an Accessibility Consultant either in person or virtually over Zoom to discuss the disability and limitations presented in the academic setting. 

The student should come prepared to the first meeting with information about accommodations that have helped in the past, the current challenges, and information about the classwork and current class demands. 

The Accessibility Consultant will work with the student to determine accommodations that might help with access to the campus and coursework. 

Step Five: Eligibility Determination

The Accessibility Consultant and/or CDA Case Management team will review the students documentation to make a determination of eligibility and the student will then be informed by email regarding their status. 

The Accessibility Consultant may request additional information or documentation.  If this is the case, the student may be approved for temporary services while additional information is being considered.

Step Six: Implement Accommodations

The student and Accessibility Consultant will discuss the approved accommodations, policies, and requirements for implementing the accommodations.  The student will then follow the Process to Implement Accommodations.
