CDA Processes and Procedures During Fall 2021

Welcome back!  Whether you were here virtually over the past year, or just joining us, we are looking forward to serving you in Fall 2021.  As we return to in-person courses and services, CDA processes and procedures have been adjusted.  The following information provides a summary of these changes.  CDA staff will work with you to help ensure continuity of accommodations.  If you have any questions, please contact us using this online contact form or call us at (801) 581-5020.  Thank you for your understanding and patience as we work to adapt processes and procedures.  

Q: How will accommodation forms be handled?

A: All paper forms, including Yellow Exam Forms, Green Notetaking Forms, have been permanently discontinued.  Accommodations will be communicated to instructors via electronic PDF memos.  These memos will be generated by your Advisor and sent via email to instructors.

Q: How can I request my accommodation memos?

A: The preferred method to request Accommodation Memos is to complete the Accommodation Request form available on the CDA Online Portal.  You may also email your advisor directly, or use the online contact form, to ask any questions regarding your accommodations.  In some instances, you may be asked to make an appointment to meet with your advisor. All CDA Advisors are available to meet virtually with students by appointment.

Q: What about exams?  Will those be available in-person?

A: Yes!  We will be providing in-person exam services.  Due to COVID restrictions, testing capacity will be significantly constrained and we may need to adjust exam dates/times based on space availability.

Q: Will CDA offer walk-in appointments?

A: CDA will no longer offer walk-in appointments.  However, we have expanded the availability of scheduled appointments with both in-person and virtual options.  In addition, while Fall 2021 classes are in session CDA will offer extended hours until 7:00pm on Wednesdays and Thursdays.

Q: Will you offer in-person or virtual appointments?

A: Both!  We recognize that for many students virtual appointments provide greater flexibility.  You may request any in-person appointment to be converted to a virtual appointment.  Please note that depending on demand, in-person appointment availability may be limited.

Q: How can I schedule a time to meet with my Advisor?

A:  You can schedule a time to meet with your advisor either by calling our office, (801) 581-5020, emailing your advisor directly or using the online contact form.  Please allow adequate time for your Advisor to respond to schedule an appointment.

Q: Will the CDA office by physically open?

A: Yes, the CDA office will be open Monday through Friday, 8:00AM to 5:00PM.  While Fall 2021 classes are in session, we will be open for extended hours until 7:00pm on Wednesdays and Thursdays.  To comply with physical distancing requirements, there may be a limit on how many individuals can enter the office at one time.

Q: Where can I find information about masks and vaccines?

A: Up-to-date information regarding vaccinations, masks, testing is available at https://coronavirus.utah.edu.  The University of Utah is a mask-friendly campus.  While masks are not required, students, faculty, and staff are encouraged to follow the CDC guidance regarding when face coverings are encouraged.  We will have free disposable masks available in our office.
