Professor/Instructor CDA Fall 2021 Information and Resources

We are happy to announce that CDA will be providing in-person exam accommodation services during Fall 2021.  We appreciate the patience as we return to in-person exam services.  There will be continued adjustments as needed and CDA staff will work with you to help ensure continuity of accommodations.  Faculty/instructors can continue to choose to directly deliver exam accommodations to students without using CDA exam services.

All previous paper based forms (Yellow Exam Forms, Green Notetaking Forms) have been discontinued. Faculty/instructors will be notified of approved ADA accommodations via an official Memo of Accommodation in PDF format identifying the approved accommodations.  In most circumstances, this memo will be emailed to the instructor's uNID@utah.edu email address from the CDA advisor with a CC: sent to the student.  If you have any questions regarding the memo, please contact the specific CDA advisor. 

Please note starting Fall 2021 in compliance with the University Employee Individual Email Policy 4-010, all CDA email communications to Faculty will be sent to the official UMail address (uNID@utah.edu).  For more information please visit New Policy Requires U Employees to Use Approved Email Systems

Exam/Quiz Accommodation Information

The Exam/Quiz Accommodation process has been modified for Fall 2021 to allow for in-person exam services.  Students receiving exam/quiz accommodations will have their approved accommodations indicated on their memo of accommodation.  Faculty/instructors will be required to complete an online exam form for each approved student as soon as possible.  This will allow us to track and coordinate the exam accommodations in a timely manner.

For exams delivered via Canvas or other test delivery platforms, instructors will be responsible for configuring those accommodations.  For information on how to configure extended time on Canvas exams, please visit this Canvas help page

Please Note: Due to COVID restrictions, our capacity will be significantly constrained and we may need to adjust scheduled exam times based on space availability.  We appreciate your flexibility in serving our students.

Instructional Guidance

The University has released updated Fall 2021 Instructional Guidelines, which contains ADA accommodation information, including a link to additional CDA guidelines related to COVID adjustments in Fall 2021.

!!Upcoming CDA Enhancements!!

During Spring and Summer 2021, CDA has begun to prepare for the implementation of ClockWork Accommodation Software system.  Once implemented, this new system will provide a centralized portal for all accommodation requests and services, including the ability for faculty to view all accommodation memos in a centralized location.  While the implementation was not able to take place prior to Fall 2021 courses beginning, once in place it will significantly streamline all services for students, faculty, and CDA staff.  We are looking forward to sharing these enhancements with you.  Select components will begin to be utilized mid-semester of Fall 2021.
