Exam Accomodations

The Center for Disability Services provides exam accommodations for students with an applicible documented disability. Students who qualify for these accommodations coordinate with their professor and the CDS to provide the support.

Professors and Instructors

As a service to instructors, CDS will administer exams to eligible students on the instructor's behalf.

When presented with an exam accommodation form from a student, instructors are responsible for completing the instructor portion of the form, which includes a list of all scheduled exam dates for the semester, the length of time in minutes that the class will receive to complete the exams, allowed items for the exam, and the preferred methods by which the exam will be delivered to CDS and returned to the department.

Exams will be scheduled in accordance with CDS policies:

• Exam Hours: Monday - Friday: 8:00 am - 4:00 pm
• Notification of a change in exam date/time must be made at least five school days prior to the exam date in order to guarantee the availability of the student's approved accommodations.
• Changes must be authorized by the instructor by writing, calling (801) 585-3877, or emailing exam@disability.utah.edu.

Students

Exam accommodations are available based on documentation of a diagnosed disability. Students qualifying for exam accommodations should meet with their CDS advisor to receive an exam accommodation form for each course for which they are seeking accommodations.

It is the student's responsibility to:

• Disclose the need for exam accommodation to the instructor or professor.
• Complete the student portion of the form.
• Meet with instructors or professors to complete the instructor portion of the form.
• Submit the form to CDS at least five school days prior to the first exam in order to guarantee the availability of all approved accommodations.
